CMS Opens Primary Care Model Applications


The Centers for Medicare & Medicaid is now accepting applications for its new primary care medical home model — Comprehensive Primary Care Plus.

CPC+ will focus on strengthening primary care through a regionally-based multi-payer payment reform and care delivery transformation, as noted by CMS, and will include two primary care practice tracks.

The program is a five-year model that is scheduled to begin in January 2017 for approximately 5,000 primary care practices across the United States.

States participating in full include:

  • Arkansas
  • Colorado
  • Hawaii
  • Michigan
  • Montana
  • New Jersey
  • Oklahoma
  • Oregon
  • Rhode Island
  • Tennessee

The program will also be offered in the following regions:

  • Greater Kansas City Region (Kansas and Missouri)
  • North Hudson Central Region (New York)
  • Ohio and Northern Kentucky Region
  • Greater Philadelphia Region
