The Centers for Medicare & Medicaid is now accepting applications for its new primary care medical home model — Comprehensive Primary Care Plus.
CPC+ will focus on strengthening primary care through a regionally-based multi-payer payment reform and care delivery transformation, as noted by CMS, and will include two primary care practice tracks.
The program is a five-year model that is scheduled to begin in January 2017 for approximately 5,000 primary care practices across the United States.
States participating in full include:
- Arkansas
- Colorado
- Hawaii
- Michigan
- Montana
- New Jersey
- Oklahoma
- Oregon
- Rhode Island
- Tennessee
The program will also be offered in the following regions:
- Greater Kansas City Region (Kansas and Missouri)
- North Hudson Central Region (New York)
- Ohio and Northern Kentucky Region
- Greater Philadelphia Region
