Course Overview
This continuing medical education (CME) is designed to equip hospitalists, advanced practice providers, and healthcare administrators engaged in the delivery of acute and post-acute care.  The course provides a comprehensive exploration of the post-pandemic evolution of hospital medicine, focusing on the management of escalating patient complexity and the shift toward longitudinal care models. Participants will examine the systemic impact of post-acute sequelae of SARS-CoV-2 (PASC), the implementation of “Hybrid Hospital” and Hospital-at-Home (HaH) architectures, and the integration of predictive analytics in daily workflows. By synthesizing recent clinical evidence with operational innovations, this course equips clinicians to optimize patient safety, functional recovery, and system-wide resilience in a modernized healthcare environment. 

Learning Outcomes 
Upon the completion of this course, the learner should be able to:

  • Recognize the multifaceted changes in patient demographics and disease burden impacting hospital medicine since the onset of COVID-19.
  • Describe the long-term sequelae of SARS-CoV-2 infection and their implications for acute and post-acute care planning.
  • Identify system-level workflow adaptations—including telehealth, interprofessional collaboration, and data-driven decision support—that improve care delivery in complex clinical environments.
  • Apply evidence-based approaches to manage high-acuity and functionally impaired patient populations within multidisciplinary care models.
  • Integrate innovative care models and predictive tools into daily clinical practice to improve throughput, safety, and quality outcomes. 

About the Author/Presenter 
Dalia Saha, MD, is board certified in internal medicine and serves as a hospice medical director. Dr. Saha received her training at Brown University and has since dedicated her career to medical education, teaching, and medical writing. With a passion for imparting knowledge and enhancing patient care, Dr. Saha brings a wealth of experience and expertise to her role, ensuring excellence in both clinical practice and educational endeavors.
